How does hormone replacement therapy with bioidentical hormones work?

BHRT in perimenopause

Hormone replacement therapy is usually abbreviated in the literature as “HRT” (= “Hormone Replacement Therapy”). When hormone replacement therapy with bioidentical hormones is specifically meant, the abbreviation “BHRT” is often used. This therapy usually consists of an estrogen medication and a progesterone medication. In my opinion, both should be bioidentical.

Perimenopause refers to the phase of life before menopause begins. Perimenopause can last for a period of 5–10 years. During this time, one could already start with bioidentical progesterone, as a cream or capsule, taken orally or vaginally. Progesterone helps very well with sleep disturbances, which is why it should be used in the evening.

BHRT in menopause

Menopause is defined as the phase of life after menstruation has been absent for at least one year. In menopause, bioidentical estrogen could be substituted in addition to progesterone. It is administered through the skin using a gel. I would also apply this in the evening because estrogen improves deep sleep.

Weigh benefits and risks in close consultation with your doctor

Although bioidentical hormones are chemically identical to the body’s natural hormones, they can still have risks and side effects. Therefore, when deciding on hormone replacement therapy, the benefits and risks should be carefully weighed individually and the decision should be made in close consultation with your doctor.

Please note:

Estrogen must never be used without simultaneously taking progesterone if you still have a uterus!
